• Managing a caseload in the community
• Undertaking social and clinical assessments
• Developing, overseeing and regularly reviewing support plans
• Mentoring, training and supervising support workers
• Full Time, Monday to Friday. We will consider reduced FTE
• Some on call but phone calls only
• Flexible, including some work from home by negotiation
• No weekends or public holidays.

• A Registered nurse with experience in community, aged care, or mental health
• Collaborative, compassionate, a wonderful communicator able to develop trusting relationships 
• Agile, adaptable and flexible with a can do attitude. 
• Well organised, excellent time management skills and comfortable with technology (InterRai training would be useful)
• Full Driver’s License
• Vaccination against COVID 19
