What you'll be doing
You'll provide effective social work services to patients and their family/whānau, and contribute as a valued member of the interdisciplinary team. Key responsibilities include:
- Psychosocial assessments, care planning and targeted interventions
- Discharge planning and mobilising supports promptly
- Active participation in MDT meetings across inpatient and community settings
- Family meetings, advocacy, and coordination with community partners
- Accurate documentation and management of patient information in PalCare
- Supporting grief and bereavement services, including staff debriefing as appropriate
- Current NZ Social Work Registration
- Minimum 4 years social work experience ideally from within health
- Confidence working within a multidisciplinary health team and advocating professionally
- Experience in aged care and / or existing relationships and confidence working with ARC providers and community service
- The ability to complete NASC assessments is an advantage
- Full, clean NZ driver's licence and being comfortable with travel across the region, including Central Hawke's Bay (fleet car provided)
