We're seeking a mid-level Social Worker to join their Family Support Team, working 20 hours per week initially, with the opportunity to increase as service demand continues to grow.
The opportunity
This role is ideal for a social worker who brings energy, efficiency, and confident judgement. Someone who can balance high quality support with realistic service boundaries as referral volume increases.
You'll work alongside two part time social workers, helping cover key service gaps (especially Mondays and Fridays) and supporting the team to deliver a sustainable level of care for more people. The roster is flexible, but Monday and Friday must be covered (5 hours per day minimum)
What you'll be doing
You'll provide effective social work services to patients and their family/whānau, and contribute as a valued member of the interdisciplinary team.
Key responsibilities include:
- Psychosocial assessments, care planning and targeted interventions
- Discharge planning and mobilising supports promptly
- Active participation in MDT meetings across inpatient and community settings
- Family meetings, advocacy, and coordination with community partners
- Accurate documentation and management of patient information in PalCare
- Supporting grief and bereavement services, including staff debriefing as appropriate
- Current NZ Social Work Registration
- Minimum 4 years social work experience ideally from within health
- Confidence working within a multidisciplinary health team and advocating professionally
- Strong prioritisation, pace and ability to hold workload boundaries
- The capability (and willingness) to mentor and support a newer graduate social worker in the team
- Experience in aged care and / or existing relationships and confidence working with ARC providers and community services is highly regarded
- The ability to complete NASC assessments
- Full, clean NZ driver's licence and being comfortable with travel across the region, including Central Hawke's Bay (fleet car provided)
