About the Job
This varied and autonomous position is largely administrative where you’ll manage a caseload and spend about 70% of your week completing online / admin tasks and the remainder visiting clients in their home completing needs assessments and reviews. Responsibilities include:
- Delivering essential healthcare services in clients’ homes
- Completing social and clinical assessments
- Developing, overseeing, and regularly reviewing care plans
- Mentoring and supervising Support Workers and Enrolled Nurses
- Collaborating with Care Coordinators and other health professionals
- Current Registered Nurse with APC
- Experience in long term conditions, aged care, or medical nursing
- Strong community engagement and caseload management skills
- Excellent communication – able to build rapport quickly and maintain trust
- Adaptable, organised, and comfortable with changing priorities
- Digitally confident – comfortable working online and offline while mobile
- Full driver’s licence
- Stable full time, Monday to Friday hours
