About the Job
This is a meaningful and rewarding community nursing role. You will manage a caseload of 30 to 40 clients aged 65 and over or under 65 with long term health conditions or mental health needs. Clients are based across Napier. Working as part of a team of six experienced community nurses and supported by a Clinical Quality Manager, you will:
- Complete InterRAI assessments and develop individualised care plans
- Conduct regular reviews and ongoing health monitoring
- Coordinate support services that uphold client independence and dignity
- Use clinical judgment to maintain safe, respectful boundaries
- Work collaboratively with internal teams and external providers
- Maintain clear, accurate documentation using both current and cloud-based systems
About You
You are a New Zealand Registered Nurse with a current APC and bring experience in aged care, mental health, rehabilitation or community health settings. You combine professionalism with strong emotional intelligence, and you understand the importance of clear boundaries while delivering compassionate, client centred care. You will thrive in this role if you:
- Are confident using technology and able to manage documentation across multiple systems
- Communicate clearly and value collaboration within a supportive team
- Respond well to feedback and demonstrate a flexible, solutions-focused mindset
- Are organised, values driven, and committed to delivering high quality, person centred care
- $45 per hour
- 0.8 FTE (32 hours per week) Days and hours worked are negotiable. We will consider full time.
