Community Palliative Care Nurse

We are seeking a compassionate and experienced Community Palliative Care Nurse to join our team, delivering specialist palliative care to patients and whānau across the Wellington region. You can be based in Newtown or Porirua.
 
This is a highly rewarding community-based role where you will provide and coordinate specialist palliative care in patients' homes - supporting them and their whānau through all stages of illness, including bereavement. You'll work autonomously while being closely connected to a supportive multidisciplinary team.
 
Key responsibilities include:
  • Assessing patient and whānau needs and coordinating care
  • Delivering specialist palliative nursing care in the community
  • Providing advice, education and support to patients, carers and other health professionals
  • Working collaboratively with GPs, specialists and community providers
  • Supporting patient-centred decision making and care planning
  • Maintaining accurate clinical documentation (PalCare)
About You
  • Registered Nurse with a current APC
  • Strong experience in palliative care, oncology or community nursing
  • Confident working autonomously in the community
  • Excellent communication and relationship-building skills
  • Calm, adaptable and solutions-focused
  • Committed to culturally safe practice and Te Tiriti o Waitangi principles
  • Full NZ driver's licence
