This is a highly rewarding community-based role where you will provide and coordinate specialist palliative care in patients' homes - supporting them and their whānau through all stages of illness, including bereavement. You'll work autonomously while being closely connected to a supportive multidisciplinary team.
Key responsibilities include:
- Assessing patient and whānau needs and coordinating care
- Delivering specialist palliative nursing care in the community
- Providing advice, education and support to patients, carers and other health professionals
- Working collaboratively with GPs, specialists and community providers
- Supporting patient-centred decision making and care planning
- Maintaining accurate clinical documentation (PalCare)
- Registered Nurse with a current APC
- Strong experience in palliative care, oncology or community nursing
- Confident working autonomously in the community
- Excellent communication and relationship-building skills
- Calm, adaptable and solutions-focused
- Committed to culturally safe practice and Te Tiriti o Waitangi principles
- Full NZ driver's licence
