- Experience working with older people, preferably in aged care
- Case management including case conferencing and family liaison
- Identifying and managing clinical risk; solid understanding of the new Aged Care Standards
- Excellent team management, administration, computer and software skills
- Flexible, reliable and a “can‑do” attitude
- Minimum 3 years aged care experience in Australia
- Provide on‑site care management and ensure compliance with Aged Care Standards
- Lead clinical assessments, individualised care plans, medication safety and incident management
- Coordinate case conferences and liaise with families, GPs and allied health
- Maintain accurate clinical records and support continuous improvement
