About the role:
You’ll be part of multidisciplinary teams providing support through telehealth, home visits, in-centre consultations and mobile outreach. Referrals come through NDIS, Medicare and private channels. When you're not seeing clients, you’ll have flexibility to work remotely or from one of the organisation’s hubs.
What you'll be doing:
  • Supporting clients of all ages with varied and meaningful needs
  • Encouraging independence, wellbeing and enhanced quality of life
  • Delivering interventions, therapies and assistive technology recommendations
  • Collaborating within multidisciplinary teams to deliver holistic outcomes
  • Managing a varied caseload with full team support
  • Documenting with accuracy and intention
  • Building trusted connections with clients, families and referrers
About you:
  • Degree-qualified and registered with AHPRA or your relevant professional body
  • Driven by connection, client outcomes and continued growth
  • Open to learning, particularly if you're at the beginning of your career
  • Driver’s licence and reliable vehicle
  • Relevant checks: Working with Children, NDIS Screening, Police Check (international if applicable)
  • Professional Indemnity and Public Liability insurance
